<<app>> is no longer avilable

One of our custom ios apps, that we have installed on multiple devices
internally, as of this morning is either crashing without message or
putting up the message that the app is no longer available.  If I attempt
to uninstall and reinstall it, no joy.  I check the provisioning profile,
and it is current.
