IconBuilder is fairly finicky about its host app.  While Fireworks also 
support most Photoshop plugins, IconBuilder doesn't seem happy with 
anything other than PhotoShop.

I think I found a good solution -- and it's free:

IconComposer is included in the dev tools with OS X.  To make those into 
XP icons I just found icon2ico at VersionTracker:

It seems to do a good job of making the .ico with drag-and-drop 
convenience.  Now to go see how they look on Win....
