Installing 4.0 on OS X gets error 565.1994

Hmmm... maybe permissions? Try installing under the root user account. Even before that, I would definitely check your hard drive for any directory errors before proceeding, and run Repair Permissions as well. Sounds like I'm a lowbie tech support guy reading from a script, but I would eliminate those things as possible causes first.
