Add file to Custom Property

Now for the encryption...
base64 is not encryption.  It is conversion to another character  
representation, which is easily transparent to most word processors,  
such as BBEdit, TextEdit, etc.

You need to decide what level of security is required.
Read up, (especially if medical data is involved)
