On page 4, using the top button for PDF, my experience was exactly the opposite of yours: it worked first time.
However, the first time I used the bottom button for Flash, it crashed. But it worked at the 2nd attempt.
After that, I tried to navigate backwards to a previous page, and Rev got stuck in an infinite loop.

The report only refers to the page 1 problem.
